That's messed up, but I'll honestly say I wasn't grossed out. In person, though, who knows. I've seen some pretty bad things before and have shadowed surgeons in the OR so maybe I'm a bit desensitized.

That being said, from a medical point of view, I have never seen or heard of anything like this before. As horrific as it sounds, falling face-first was what prevented him from dying on the spot (although supposedly he perished hours to days later). The muscle/anterior bone structure of the face absorbed the force of impact and saved him from severing his brainstem. Hard to say if any/how much of his cortical brain is intact - it's possible that he destroyed his prefrontal cortex from the impact.


I was also shocked by the quality of medical care. Why was he intubated but not put on ventilator? Why was he not sedated in the ER? Etc etc

Dont think it is even possible to repair that. Initally when the accident happened he did lose a lot of blood in the ocean water. secondly the wound is too big near vital organs. Even if you can transfuse him with blood there are too many arteries and veins broken. Say you start with the brain and stitch your way down that procedure would take way too long for him to even survive. But then again i am no surgeon so i dont think i can say its impossible, but i would probably imagine a whole team of specialist giving them the challenge of their life.

I agree with you, but the fact that he lived a few days with substandard healthcare makes me wonder what would have happened with some better doctors and modern equipment treating him.

Good question. It's extremely hard to say. The fact that he seemed to have some respiratory/autonomic motor function intact suggests that his brainstem was not severed or severely destroyed (see how is he moving in the video?) More importantly, it means he was not totally ischemic (lack of blood flow) as of that time. All of those signs are promising.

Survival would depend on 1.) infusing enough fluid/getting him on a respirator so that he does not go into shock from blood loss, 2.) rapid closure of major arterial/venous bleeds 3.) restoring blood flow to his brain RAPIDLY to prevent brain death and 4.) infection prevention.

1 and 4 are pretty basic to any medical center, except out in the wilderness or in awful quality hospitals. 2 and 3 are tricky - you need a highly, highly skilled vascular surgeon/trauma surgeon /neurosurgery team to fix these.

So, my long winded reply is...depending on the damage already done prior to getting to the hospital, he could have survived in a better medical center. But either way this is not an easy case by ANY means. Hope this was informative!

A skilled ENT/plastic surgeon could fix the structural damage. But as you correctly pointed out, it's the blood loss that's more concerning
