E60 M54 rough startup

Hey guys, I know it's a e39 forum but my mom's car is a E60 530i with the M54, I thought you guys would be more familiar with the M54.

Everytime you start her car it shakes very hard and settles down slowly in around 30secs, I've replaced the pcv system, plugs, coils, oil and coolant. The car runs perfectly smooth once it has settled down, anyone have an idea what the issue could be? Thanks!

I also have an E60 with M54 3.0 liter, and know the M54 well from several cars.
Shaking could be sticky Vanos. If no CE codes flip or EML light comes on, then replace all the vacuum lines, ICV + rubber sleeve, and if needed, intake boot.
Plug into a data reader and logger to observe what's going on with spark advance and Vanos behavior and verify all injector activations. Good luck!
